Overview

When you enroll for certain plans through Nutanix, you'll receive an ID card or debit card. ID cards should be shared with your provider whenever you receive healthcare services. Debit cards can be used to pay for eligible expenses.

Medical When you enroll in a new medical plan, you’ll receive a new physical ID card. Cards should arrive within 30 days of completing enrollment if you’re a new hire, or in January if you make changes during the annual open enrollment period in the fall. You are able to access a virtual card through the medical plan app at any time. Employees enrolled in a Kaiser Permanente plan will also receive a travel card, which contains information on where to get urgent care while traveling outside the Kaiser network area. 
Prescriptions If you enroll in a UnitedHealthcare medical plan (all locations except Hawaii), you'll receive a separate ID card from Express Scripts. If you enroll in the Kaiser Permanente HMO or the UnitedHealthcare plan in Hawaii, you will not receive a separate prescription card—your medical ID card is for prescription drug purchases, too.
Dental When you enroll for the first time, or if you have an address change, you’ll receive one ID card from Delta Dental to use for all covered members.
Vision You won’t receive an ID card for your vision benefits. Instead, you can access your benefit information on the VSP website or you can give your name and the last four digits of your Social Security number to your provider so they can access your coverage.
Flexible Spending Accounts (FSAs) and Health Savings Account (HSA) Once your account is created with HealthEquity, you can register at www.healthequity.com/wageworks. A welcome packet with your debit card (commonly referred to the HealthEquity Stacked Card) will arrive within within 30 days of your enrollment.

Note: If you have multiple accounts with HealthEquity, like an HSA and the Limited Purpose FSA, the HealthEquity Stacked Card will deduct funds from the Limited Purpose FSA before using funds in your HSA.
Health Reimbursement Account (HRA) (Kaiser Permanente HMO)

Once your account is created with HealthEquity, you can register at www.healthequity.com/wageworks.

Note: You will not receive a debit card for the HRA. All expenses related to Kaiser Permanente HMO must be reimbursed through the HealthEquity portal or mobile app. To obtain HRA reimbursement, you'll need a detailed itemized receipt from Kaiser or a Kaiser explanation of benefits (EOB) after your visit.

Legal Insurance and Identity Theft Protection When you enroll in the ARAG Legal Insurance and Identity Theft Protection plan, you'll receive a welcome packet and ID cards the month following your enrollment date. You can utilize services the first of the month following your enrollment by logging into your ARAG account.
