Leave of Absence
- If you are or will be out of the office for more than 5 business days due to your own illness, injury, pregnancy, care of an ill family member, birth/adoption/placement of a child or military leave, you must report a leave.
- Review our leave of absence programs.
- To get started, call Prudential, our leave of absence administrator, at 1-877-367-7781 to request your leave and apply for short-term disability, if applicable (control number 71041).
- Submit a leave of absence claim to Prudential online (control number 71041)

- 30 days prior to your leave start date, or as soon as reasonably possible, notify loa@nutanix.com and your manager of your last day worked, first day of absence and anticipated return date (we understand the return date may change).
- Review the Employee Assistance Program (EAP) through Concern (access code: Nutanix).
Injury on the Job
- If this is a medical emergency, please dial 9-1-1.
- If this is not an emergency, please contact security at 408-840-2595.
- Notify your manager immediately.
- E-mail LOA@nutanix.com — you will be asked to complete an incident intake form, other forms and file your claim with the workers’ comp carrier.
- Seek medical care at an authorized occupational clinic for evaluation and treatment (non-emergency).
