The loss of income due to illness or injury can cause serious financial hardship for your family. Disability insurance, provided through Prudential, replaces a portion of your income so you can continue paying bills and meeting financial obligations during this difficult time.

If you’re a full-time employee who works at least 30 hours a week, you’re eligible for short-term and long-term disability insurance coverage.
Company-paid coverage that pays you a disability benefit for up to 26 weeks.
Company-protected coverage that protects you from the financial impact of an extended disability.
In order to qualify for disability benefits, you’ll need to submit an application to Prudential and provide medical certification at the time of the disability. Benefits will begin after your application is approved by Prudential.

You choose whether the benefit you receive is taxable or not taxable. Here’s how each option works:
With Option 2, the value of the premium is imputed income.* Imputed income is the value of the STD benefit paid by Nutanix on your behalf, and it increases the taxable income reported on your W-2. If you don’t want to pay imputed income on the STD premium that Nutanix pays on your behalf and you would rather have your STD benefit payments taxed, you must actively elect Option 1.
You choose how your STD benefits are taxed when you first enroll in benefits or during Open Enrollment each year. If you don’t make an STD tax choice, you’ll be automatically enrolled in Option 2 in Workday.
*Imputed income is the value of the Short-Term Disability benefit paid by Nutanix on your behalf.

Elimination period | Benefits begin after 180 days of disability.* |
Amount of the benefit | Benefits equal 60% of your monthly base salary, up to $10,000 per month. |
Benefit maximum | Benefits continue until you’re no longer considered disabled or you reach normal retirement age, whichever comes first. However, there is a lifetime maximum of 24 months of coverage for long-term disabilities resulting from a mental illness or alcohol/substance use disorder. |
Taxation of benefit | Benefits are taxable. |
*Pre-existing conditions aren’t covered in the first 12 months from the effective date of coverage.
