Nutanix provides you with Basic Life and AD&D insurance through Prudential so you can protect those you love from the unexpected.

AD&D benefits are paid in addition to any life insurance if you die in an accident or become seriously injured or physically disabled.

For added protection for you and your family, you can also purchase Voluntary Life and AD&D insurance.

Note: Federal tax law requires Nutanix to report the cost of company-paid life insurance in excess of $50,000 as imputed income.

 

Basic Life and AD&D Insurance

Sometimes the unexpected happens, but you can have peace of mind knowing that your Nutanix benefits have your family covered. Your basic life and AD&D insurance is provided through Prudential at no cost to you.

Benefits are equal to:

  • Company-paid life insurance: 3x your annual base earnings up to $1,000,000.
  • Company-paid AD&D insurance: 3x your annual base earnings up to $1,000,000.

What is AD&D insurance?

Should you lose your life in an accident, or if you lose your sight, hearing, speech, or use of your limb(s) in an accident, AD&D insurance provides additional benefits to help keep your family financially secure. AD&D benefits are paid as a percentage of your coverage amount—from 50% to 100%—depending on the type of loss.

 

Employee-paid Life and AD&D Insurance

You can purchase additional life and AD&D insurance for yourself, as well as coverage for your spouse and your child(ren). You pay the full cost of this additional coverage.

You may elect the following:

  • Employee Voluntary Life and/or AD&D coverage in $10,000 increments. The maximum is the lesser of 6X your annual earnings or $1,000,000.
    • The guaranteed issue amount for Voluntary Life is $500,000 for new hires. All elections above this amount require an Evidence of Insurability (EOI) medical questionnaire.*
    • All election levels or increases to coverage during Open Enrollment, or at a Qualified Life Event, are subject to an EOI medical questionnaire.*
  • Spouse Voluntary Life and/or AD&D coverage up to 100% of employee coverage amount in $5,000 increments. The maximum is the lesser of employee’s Basic and Voluntary Life/AD&D amount or $500,000.
    • The guaranteed issue amount for spousal life is $100,000 for new hires.* All elections above this amount require EOI medical questionnaire.*
    • All election levels or increases to coverage during Open Enrollment, or at a Qualified Life Event, are subject to an EOI medical questionnaire.*.
  • Child Voluntary Life and/or AD&D coverage up to 100% of employee coverage amount in $2,000 increments. The maximum is $20,000.
    • No EOI medical questionnaire is required for Child Voluntary and/or AD&D coverage.

You do not need to purchase coverage for yourself in order to purchase it for your dependents.

*You may have to complete an evidence of insurability (EOI) medical questionnaire to determine whether you or your spouse is insurable for supplemental life insurance amounts. If EOI is required, the form will be emailed to you by Prudential at your work email address.

Have you named a beneficiary?

It’s important to designate a beneficiary to receive the benefit paid by a life insurance policy. As personal circumstances change, be sure to keep that information up to date. Visit Workday to add or change a beneficiary.
