Nutanix provides you with Basic Life and AD&D insurance through The Hartford so that you can protect those you love from the unexpected.

AD&D benefits are paid in addition to any life insurance if you die in an accident or become seriously injured or physically disabled.

For added protection for you and your family, you can also purchase Voluntary Life and AD&D insurance.

Note: federal tax law requires Nutanix to report the cost of company-paid life insurance in excess of $50,000 as imputed income.

Basic Life and AD&D Insurance

Nutanix provides Basic Life and AD&D insurance to assist you and your family in the event of a death or dismemberment. These benefits are fully paid by the company.

Benefits are equal to:

  • Company-paid life insurance: 3X your annual base earnings up to $1,000,000
  • Company-paid AD&D insurance: 3X your annual base earnings up to $1,000,000

Should you lose your life, sight, hearing, speech, or use of your limb(s) in an accident, AD&D insurance provides additional benefits to help keep your family financially secure. AD&D benefits are paid as a percentage of your coverage amount — from 50% to 100% — depending on the type of loss.

Employee-paid Life and AD&D Insurance

You can purchase additional life and AD&D insurance for yourself, as well as coverage for your spouse and your child(ren). You pay the full cost of this additional coverage.

You may elect the following:

  • Employee Voluntary Life and/or AD&D coverage in $10,000 increments. The maximum is the lesser of six times your annual earnings or $1,000,000. The Guaranteed Issue amount for Voluntary Life is $500,000 for new hires.*
  • Spouse Voluntary Life and/or AD&D coverage up to 100% of employee coverage amount in $5,000 increments. The maximum is the lesser of employee’s Basic and Voluntary Life/AD&D amount or $500,000. The Guaranteed Issue amount for spousal life is $100,000 for new hires.*
  • Child Voluntary Life and/or AD&D coverage up to 100% of employee coverage amount in $2,000 increments. The maximum is $20,000.

You do not need to purchase coverage for yourself in order to purchase it for your dependents.

*You may have to complete an evidence of insurability (EOI) medical questionnaire to determine whether you or your spouse is insurable for supplemental life insurance amounts. If EOI is required, the form will be emailed to you by The Hartford at your work email address.
